Overview The PRN Pool Social Worker when called upon, helps people cope with the challenges they are facing in their lives. He/she will actively participate in coordinating all aspects of the patient’s care and is a client advocate and adviser. This person also assists with the ongoing interdisciplinary comprehensive assessment, helps develop a plan of care and contributes to educating the patient and their family while giving them counsel as well. This person participates in the QUAPI program and hospice sponsored in-service training. Location: Falls Church, VA - Headquarters Schedule: PRN COVID-19 vaccine required to be completed upon start. Responsibilities The Social Worker, PRN POOL, when assigned, is an active member of the Capital Hospice Interdisciplinary Team. He/she provides a wide variety of direct and indirect social work services to patients, caregivers and families in accordance with the individualized plan of care, as prescribed by the physician. Qualifications Licensure Must be licensed or license eligible in compliance with jurisdiction of practice (LCSW, LPC). Education Must have a Master's Degree in Social Work from school accredited by The Council on Social Work Education. Experience Two years of master’s level experience in an interdisciplinary health care setting desired.
