REQUIRED QUALIFICATIONS:

• Master’s Degree - Social Work from a CSWE Accredited School of Social Work, OR from a school in candidacy status by the council of Social Work Education (CSWE).
• Oregon Clinical Social Worker License upon hire.
• Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.
• 2 years - Relevant Social Work experience. May include CSWA experience.

PREFERRED QUALIFICATIONS:

• Care Management: Experience in acute care settings.
