As the Admissions Coordinator, you will provide admissions coordination and social work services, assessing applicants and placement of resident at the Minneapolis Veterans Home - a skilled nursing facility located on a 53-acre campus. Position will also provide clinical support and analysis to assure residents needs are addressed, carrying out planning and implementation of social services for residents of the Home.

Duties include but no limited to the following:

  • Lead and manage the admissions process through active recruitment of residents, conducting tours, scheduling and facilitating meetings, and intake process
  • Responsible for the resident’s move-in, transition process, integration, transfer, and discharge – through comprehensive assessment and resolving issues that may arise
  • Maintain current knowledge of state and federal statutes, rules, and policies, resident rights through training, conferences, and work events
  • Complete all documentation and pre-admission screenings/assessments, manage applicant wait-list, and reports/files for compliance with state, federal, and agency regulations
  • Serve as the outreach and community services liaison for residents for accessibility, build rapport and support with stakeholders, host and participate in community events, outreach programming, educational seminars while providing resources and education of the admissions process

MINIMUM QUALIFICATIONS

  • Current licensure in Minnesota as a Licensed Social Worker (LSW) or ability to obtain licensure by date of hire.
  • One year experience as a Social Worker.
  • Skills in completing social work assessments sufficient to evaluate clients’ social service needs.
  • Human relations skills sufficient to develop and maintain effective working relationships with other staff, residents, resident representatives, and community groups.

PREFERRED QUALIFICATIONS

  • Experience in a long-term care or skilled nursing care facility.
  • Experience working with Veterans.
  • Knowledge of federal and state regulations related to skilled nursing facilities.
  • Experience in electronic charting systems such as PointClickCare.

ADDITIONAL REQUIREMENTS

  • Minnesota Department of Human Services (DHS) background check may be conducted on the top finalist(s) for the position(s). When conducted, a finalist must pass the DHS background check.
  • A Tuberculosis (Mantoux) screening (or chest x-ray) is required prior to employment.
  • Employment reference checks will be conducted on all finalists. This may include a review of documentation related to job performance. It also includes contact with the applicant’s former employer(s).
