LOOKING FOR A FULL TIME LICENSED SOCIAL WORKER FOR NAVIGATOR HOMES

JOB SUMMARY

Assists in meeting the psychosocial needs of residents in a long-term care facility. Collaborates with the inter-disciplinary team to develop and carry out individualized care plans to enable each resident to achieve their optimal level of health and well-being and maximize their quality of life.

MA LCSW REQUIRED.

• A minimum of 2-3 years’ experience in a long-term care setting.
• Solid working knowledge of discharge planning, Medicare/Medicaid, OBRA regulations, community resources, social work methods and techniques.
• Excellent communication, interpersonal, and computer skills required.
• Knowledge of electronic medical records, MDS, and Care Plans.

We are loooking for compassionate dedicated indivduals to join our team as Director of Nursing, leading and managing Universal Workers, in a unique role that goes beyone tranditional caregiveng. With just 14 residents per home, this is not a facility, it’s a family!

EXPERIENCE:

• Skilled Nursing Facility: 2 year (Preferred)
• Long-Term Care: 1 year (Preferred)
• Supervisory: 2 year (Preferred)
