The Alzheimer Society of Sarnia-Lambton (ASSL) is a community support service organization that provides programs and services to people living with dementia and their care partners in Sarnia-Lambton. We work closely with other service providers to ensure quality care and support for our clients.
We employ administrative and fund development staff, educators, social workers, nurses, therapeutic recreation staff, personal support workers and volunteers who work together as a team to meet the needs of people living with dementia and their care partners.

Reporting to the Manager of Clinical Services, the Social Worker is responsible for providing ongoing support to care partners of those living with dementia through individual and group counseling, education, care planning and service coordination.

EDUCATION:

  • Bachelor’s degree in social work from an accredited university with current registration with the Ontario College of Social Workers and Social Service Workers. Master’s level education is preferred
  • Further study in the field of gerontology, dementia is an asset 

EXPERIENCE:

  • Experience working with persons living with Alzheimer’s disease or related dementias is preferred
  • Experience in the health care and/or social service sectors is considered an asset

OTHER KNOWLEDGE, SKILLS, ABILITIES OR CERTIFICATIONS:

  • Ability to communicate, orally and written, in French is preferred.
  • Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
  • Knowledge of Alzheimer’s disease and other dementias
  • Knowledge and adherence to OCSWSSW Code of Ethics
  • Knowledge and understanding of all pertinent legislation and policies, including the Personal Health Information and Protection Act (PHIPA, 2004)
  • Ability to conduct needs and risk assessments and ability to respond in emergency situations
  • Knowledge of case management, case conferencing and intervention strategies
  • Competency related to diversity and oppression
  • Knowledge of internal and external community resources
  • Flexibility and adaptability to competing needs and various work environments
  • Excellent organizational, verbal and written communication skills
  • Excellent interpersonal skills
  • Proficient computer skills

TRAVEL REQUIREMENTS:

  • Frequent travel in and around Lambton County to carry out the functions of this position, and to attend relevant meetings. Occasional travel outside of Sarnia-Lambton may be required. Valid driver’s license and access to a dependable vehicle are required
