Description

The Social Worker is accountable for the delivery of social work services including care coordination, planning, concrete and support counseling, entitlements and other community resources regarding chronic disease management. Provides consultation, education, and other services as requested by the clinicians within the Department, in the Hospital or in the community. Provides social work services in accordance with National Association of Social Worker (NASW), Case Management Standards and the Hospital’s mission and goals. Qualifications * Master’s Degree in Social Work * Previous hospital work experience - Required * Discharge planning experience - Preferred * New York State License Masters Social Worker (LMSW) * Broad knowledge of community resources and social work / case management regulatory requirements. * Knowledge of computer and computer applications – word processing, spreadsheets and clinical application software – Preferred * Knowledge of evidence-based therapeutic techniques. * Ability to handle high stress situations
