ABOUT THE TEAM

We are looking for a passionate Transitions Social Worker to join our Specialist Adult Social Care services, which are based in Keynsham. This role is a specific role within the transitions team, working with young people (16 to 25) with the appearance of social care needs transitioning into adult care.
As a social worker you will be responsible for carrying out the responsibilities namely: to undertake first adult care assessments and reviews for autistic young people and those presenting with mental health needs, and their carers, and to work with service users to create care and support plans which meet their needs.
You will join a friendly and extremely supportive team of Social Worker professionals, giving you the opportunity to work within a multi-disciplinary role liaising with other partners and agencies, including Avon and Wiltshire NHS Partnership Adult Mental Health Teams, CAMHs, SEND Practitioners, Children’s Services, Psychologists, Occupational Therapists, Speech and Language Therapists and education professionals.
About the Role
As a Transitions Social Worker you will manage a caseload which will include complex cases and will work independently and effectively, under the general supervision of a Team Manager.

You will carry out responsibilities including:

  • Undertaking first adult care assessments and reviews of people with care and support needs and carers in accordance with the principles of the Care Act 2014
  • Creating care and support plans which appropriately meet the needs of people with care and support needs and carers.
  • Undertaking Mental Capacity assessments and, where appropriate, complete Best Interests Decisions in accordance with the Mental Capacity Act 2005.
  • Undertaking risk assessments and work to reduce risk or engage with the person and MDT in well considered positive risk taking strategies.
  • Completing S117 Aftercare reviews and Continuing Health Care Checklists where there is an identified need.

ABOUT YOU

  • Be a qualified Social Worker (Social work degree, Diploma in Social Work, CQSW or equivalent).
  • Ideally you will have had experience working with wider range of young people and adults including people with physical disability, Autism, Mental Health Conditions or Learning Disabilities.
  • Registered with Social Work England.
  • Have a Full UK Driver’s License and access to a car due to business need.
