In this role, you will provide social case management to patients and families experiencing challenges that impact their health and well-being. You will collaborate with physicians, nurses, and community agencies to ensure continuity of care and help patients achieve optimal health and social adjustment. Responsibilities include assessing social problems, planning interventions, and connecting patients with community resources. You will also advocate for patients, participate in case conferences, and educate staff on the relationship between emotional and physical health. This position requires strong communication, leadership, and problem-solving skills to deliver exceptional patient-centered care. Requirements: Education: Master’s degree in Social Work. Licensure: Valid Indiana Social Work license. Experience: Minimum of one year in a medical or mental health setting with demonstrated leadership ability. Strong verbal, written, and listening skills. Ability to collaborate effectively with healthcare teams and community agencies. Proficiency in computer applications and critical thinking skills.
