Do you thrive and enjoy supporting those in need and their families? As a Social Worker, you will be an integral member of the multi-disciplinary team at Ty Grosvenor. You will be focusing on providing outstanding social work care and will work alongside service users who have mental health needs or/and personality disorders.

You will be responsible for undertaking comprehensive assessments and handling service user’s reports, ensuring their social history is available and updated. You will also prepare reports for Mental Health Review Tribunals and Hospital Managers appeals and attend hearings in collaboration with Social Services Departments.

You will be working alongside the team at Ty Grosvenor, a rehabilitation service for people with mental health needs and/or personality disorders. The service is situated in the centre of Wrexham, wales and is in close proximity to A483 road and train stations.

Your responsibilities

.

  • When it is part of an agreed care plan focussed casework with individual Service Users using a range of interventions under supervision.
  • With the Service Users permission initiate visit and maintain contact with service user’s families and/or significant others offering support, information and where relevant to the care plan joint family and Service Users sessions.
  • To take a leading role in promoting anti-discriminatory practice in a way that links the Service Users individual circumstances to the broader, social, political legal and cultural context (with relevance to care plan).
  • To act as Appropriate Adult under Place and Criminal Evidence Act 1984
  • To practice within the Vulnerable Adults Policy.
  • To play a co-ordinating role in the process of section 117 Mental Health Act 83 aftercare plans with the relevant community agencies which meet the needs of the Service Users and takes full account of risk factors.
  • In conjunction with the responsible Social Services Department or Integrated NHS, assist in the assessment of suitable after care placements working across professional boundaries.
  • To request allocation of a Social Supervisor from Social Services Departments or Probation Service for conditional discharge of Service Users.
  • To liaise with Social Services Departments and integrated NHS Trusts to ensure that reports and attendance by a representative are available for Mental Health Reviews Tribunals.

Responsibilities

  • Knowledge of relevant legislation.
  • Ability to deal sensitively with stressful and painful situations for Service Users, relatives and carers.
  • Good verbal and written communication skills.
  • Ability to assimilate complex information;
  • Understanding of Anti-Discriminatory Practice.
  • Diploma in Social Work, CQSW, CSS or other recognised qualification.
  • Experience of working with people who have Mental health problems
  • Experience of working within a multi-Disciplinary Team in Community or Hospital setting
  • Working within the Framework of the Care and Treatment Plan or Care Programme Approach.
