This position works closely with an interprofessional care team to coordinate care and improve outcomes for a small panel of high-risk patients. This is a hybrid role (50% field-based, 50% remote) focused on longitudinal care management. Standard working hours are Monday through Friday, 8:00 AM–5:00 PM, with flexibility as needed to meet patient care needs. ---------------------------------------- DUTIES AND RESPONSIBILITIES * Serve as the primary social work contact for an assigned panel of high-risk patients, completing comprehensive psychosocial assessments. * Collaborate closely with interprofessional care teams to deliver psychosocial interventions and case management services. * Provide compassionate end-of-life care support for patients and families. * Deliver longitudinal care management to support patient stability and improved outcomes. * Build and maintain trusting relationships with patients to support effective care planning. * Identify and address barriers to care through SDoH screening, referrals, and connection to community resources. ---------------------------------------- REQUIREMENTS * Master’s degree in Social Work (MSW) or Licensed Clinical Social Worker (LCSW) in the state of Florida. * Strong clinical judgment and case management expertise. * Ability to meet Duo Health care team requirements, including background check, drug screening, and COVID-19 vaccination requirements. * Reliable transportation with a verifiable good driving record; ability to travel to patient homes. * Self-motivated with the ability to work independently while following established operating procedures. * Demonstrated ability to build inclusive, equitable relationships with patients, caregivers, and colleagues. * Experience working with diverse communities and community-based resources. * Strong written and verbal communication skills. * Comfort interacting with patients and care teams both telephonically and in person. ---------------------------------------- COMPETITIVE BENEFITS PACKAGE * Comprehensive medical, dental, and vision insurance. * Generous paid maternity and paternity leave. * 401(k) plan with company matching. * Four (4) weeks of paid time off and twelve (12) paid holidays. * Access to free wellness and personal development courses. ---------------------------------------- A MISSION-DRIVEN WORKPLACE Join a patient-centered team committed to improving health outcomes and advancing health equity through innovative, relationship-based care.
