Summary:
The Home Health/Hospice Director is responsible for independently directing and managing the clinical services for the Home Health & Hospice Programs in assigned geographic area ensuring that patient care is initiated timely, follows standards of clinical practice, and documentation is compliant and entered appropriately in the medical record/EMR. The Home Health Director has high level knowledge of HH and Hospice pertaining to CMS and State DOH regulations and standards. He/She provides administrative oversight to hire, train, and evaluate performance and competency of staff while monitoring productivity, efficient processes, and optimal reimbursement. With the guidance and consultation of the Service Line Director, the HH/Hospice Director analyzes and manages QAPI program according to CMS standards and participates in performance improvement activities for his/her regional area. The Director also serves as the Administrator for Home Health & Hospice agencies for the purpose of the PA State Department of Health and has overall responsibility for daily operations for Home Health & Hospice, and has knowledge and participates in billing/claims/revenue cycles and financial improvement activities.

QUALIFICATIONS:

  • Education: Bachelors Prepared Registered Nurse with current PA licensure or MPT/DPT with current PA licensure.
  • Experience: Minimum of 5 years proven leadership experience in Home Health &/or Hospice with at least 3 years in a leadership/management role

Required skills:

  • Demonstrated competency & knowledge with OASIS, HH and Hospice COPs, and PA DOH regulations and licensure, and survey knowledge/experience.
  • Complies with and promotes accepted professional standards and best practice for QAPI and all performance improvement activities for Quality-Safety-Service.
  • Has demonstrated critical thinking ability, problem solving skills in an interdisciplinary setting.
  • Effective communication skills.
  • Ability to manage time effectively and prioritize projects.
  • Must be a licensed driver with an automobile that is insured in accordance with the state and/or organization requirements

Preferred Skills: Experience with start-up operations, licensure and certification for HH and/or Hospice agency.
