Job Description Special Populations Care Manager The Special Populations Care Manager is responsible for coordinating the care of high-risk SALUD patients with an emphasis on those enrolled in the Accountable Care Organization. The aim of this position is to improve patient health outcomes, reduce health care costs and enhance the patient experience for high-risk SALUD patients. The primary role will be to identify patients for care management, coordinate outreach to these patients, and develop comprehensive care plans addressing the individual needs of each patient to ensure patients receive the right care, at the right time in the right setting. We offer: Starting wage is $20 to $23 per hr. The Special Populations Care Manager will work in a team-based approach, interacting with various staff members across disciplines, as well as supporting patients and their families to facilitate self-management, self-efficacy, and sustainable behavior change. The Special Populations Care Manager will work with the Program Manager to oversee the implementation of Complex Care Plans across SALUD clinics as well as to collect, analyze, and report on monthly outcomes. The Special Populations Care Manager will support the care coordination activities of peer Care Managers throughout the Salud system whenever necessary. Qualifications Education: Bachelor's Degree in social work or a health-related field is strongly preferred High school diploma or GED, required Experience: Experience in care management, patient navigation, or social work preferred Experience in a community health setting or with underserved populations preferred Licensure/Certification: None needed Knowledge, Skills, and Abilities: Bilingual English/Spanish required Commitment to working with the underserved Sensitivity to low-income and ethnic minority communities is a must Ability to collaborate with an interdisciplinary healthcare team, including primary care providers, medical support staff, behavioral health workers, and other healthcare professionals Knowledge of community and health resources and the application process Excellent data management, organizational, and documentation skills Good verbal and written communication skills Must work well with the patient care team members Computer knowledge and skills, including proficiency in Microsoft Office programs (Access, Excel, PowerPoint, Word) Willingness to learn new software programs and work processes Self-starter, able to work independently and flourish without extensive supervision Detail-oriented, able to coordinate and prioritize multiple responsibilities The application window is anticipated to close by 12/31/25 Additional Information Salud Family Health is an Equal Opportunity Employer.
