We are seeking an enthusiastic and proactive nurse to join our team at Snapdragon Children Centre. In this role, you will collaborate with two other special school nurses to provide holistic, child- and family-centred care to children and young people with Special Educational Needs and Disabilities (SEND), Profound and Multiple Learning Disabilities (PMLD), and complex health needs, aged 4 to 19 years. This is a dynamic and varied role that includes the following key responsibilities:
Assessment, Planning, and Delivery of Clinical Care : Conduct comprehensive assessments, develop individualized care plans, and deliver high-quality nursing care tailored to the needs of students, ensuring safety and effectiveness within the school setting.
Specialist Training Delivery : Provide specialist training to school support staff, ensuring they are equipped to support students’ medical and care needs effectively.
Multidisciplinary Collaboration : Work closely with other healthcare professionals to ensure continuity of care, attending multidisciplinary team meetings and child protection case conferences as required.
Family Support and Referrals : Offer support and guidance to families, signposting them to relevant specialist services and making referrals to appropriate care providers.
Mentorship and Student Nurse Support : Mentor and support student nurses, providing guidance and supervision in school settings.

JOB SUMMARY:

The Band 5 Special School Nurse will be responsible for delivering high-quality nursing care to children and young people with Special Educational Needs and Disabilities (SEND), including those with Profound and Multiple Learning Disabilities (PMLD) and complex health needs, within a school environment. The post-holder will be part of a dynamic and supportive community children’s nursing team, working closely with other healthcare professionals, teachers, and families to provide holistic care tailored to each child’s needs.

Responsibilities

Provide nursing care and support to children and young people with SEND, PMLD, and complex health needs.
Develop, implement, and review individual care plans in partnership with children, families, and multidisciplinary teams.
Supporting school staff in administering prescribed medications, monitor health conditions, and ensure appropriate interventions in response to healthcare needs.
Monitor and manage chronic health conditions within the school setting, including epilepsy, respiratory conditions, following the student’s care plan.
Deliver health education and health promotion activities to students, staff, and families, ensuring effective communication of health-related information.
Participate in multi-disciplinary team meetings to ensure integrated care planning and delivery.
Provide support and advice on infection control and the management of health emergencies within the school setting.
Being aware of safeguarding procedures and adhering to them.
Contribute to the training and development of school staff in relation to healthcare needs of students, first aid, and managing health conditions.
Maintain accurate and up-to-date records, adhering to confidentiality and data protection regulations.
Participate in ongoing professional development and reflect on personal practice to ensure the highest standards of care.
