You will be a Specialist Community Public Health Nurse (SCPHN) Health Visitor (HV) within the integrated 0 to 19 Torbay Partnership and provide the expertise, knowledge and resources to deliver the Healthy Child Programme (HCP) to families living within Torbay.
Working with the 0 to 19 Torbay workforce to ensure pathways are followed in supporting children, young people, families and carers. The SCPHN-HV works autonomously and collaboratively to deliver a family centred public health service, taking responsibility for a defined population in partnership with the wider multi-disciplinary and inter-agency teams.
Please note post is a part time post working predominantly with families who require statutory intervention.

The post holder will work closely with the 0 to 19 Torbay teams in providing joined up care to clients
The post holder will signpost staff to wellbeing at work services including counselling/coaching.
The post holder will remain professional when dealing with highly emotional situations
Attend in-house training as required to develop current role
Participate in internal audits and support quality improvements to inform practice and improve service delivery
0 to 19 Torbay is a partnership between TSDFT and The Children’s Society and Action for Children.
