Are you an experienced RMN Nurse interested with experience in dementia assessment services?
If so, then the Hillingdon Memory Assessment/Diagnosis Services have an exciting position. This is an opportunity to build your existing skill set to delivering a high-quality mental health and dementia services to local residents and their families
Our ideal candidate would be a mental health professional with a strong interest in older people and a commitment to improving services and their professional development. To monitor and co-ordinate the care of clients on anti-dementia medication, and manage a caseload of clients referred to the memory services. To collect and analyse data and produce timely reports. To act as a specialist resource for those coping with a diagnosis of dementia and to work with other team members to effect the best possible care and support systems for both users and carers of dementia services locally, In addition supporting group sessions CST and liaising with other services where joint up working is required. You will also be involved in delivering diagnosis to patients on your case load.
