We are looking for an experienced and dedicated Case Manager to join our team at Momentum Collective. As aCase Manager for Homelessness Services, you will play a vital role in supporting and advocating for individuals within the community who are experiencing homelessness or at risk of homelessness.

Responsibilities:

  • Provide culturally appropriate case management and support services to First Nations people and their and families experiencing homelessness.
  • Develop strong, trusting relationships with clients to understand their needs and goals, and provide tailored support to help them achieve stability.
  • Work collaboratively with internal teams, external service providers, and community organisations to facilitate access to housing, healthcare, employment opportunities, and other essential resources.
  • Advocate on behalf of clients to ensure they receive fair and equitable treatment when accessing services and support.
  • Conduct comprehensive assessments of clients’ needs and develop individualised care plans with achievable goals that empower clients towards self-sufficiency.
  • Maintain accurate and up-to-date case notes, reports, and client records in accordance with organisational guidelines.
  • Participate in regular supervision sessions, team meetings, training programs, and professional development activities to enhance skills and knowledge relevant to the role.
