The Sheffield Liaison Psychiatry Service are looking for outstanding clinicians to join our team.
The Liaison Psychiatry Service is currently affiliated with the Psychiatric Liaison Accreditation Network (PLAN) and are working towards accreditation. We received over 5000 referrals (365day/24hrs a day) into our service through in the last financial NHS year. Person centered care is at the heart of our practice and we work hard to support each other through the day-to-day challenges of working in this demanding role.
We are looking to recruit an enthusiastic, motivated and experienced RMN to join our well-established Liaison Psychiatry team. Our team provides a 24 hour service to the Sheffield Teaching Hospitals (STH).
We are looking to recruit a specialist nurse to work with older people, 365 days a year during core hours to support the function of the team. The specialist nurse will work across all in-patient areas of STH (including the emergency department) to provide specialist assessment, risk assessment and management plans including discharge plans.
The Liaison Psychiatry Service is a multi-disciplinary and innovative team. You will be expected to work to these accreditation standards as part of your ongoing practice.
The specialist nurse in older persons liaison psychiatry will be expected to work across 7 days during core hours (8am-6pm).
The specialist nurse will need to conduct robust triage and comprehensive biopsychosocial assessment of older adults who present with a wide range of needs including delirium, cognitive impairment and those presenting in mental health crisis.
The specialist nurse will have knowledge of working with individuals who experience ideas of suicide, self-harm, acute mental illness presentations and dementia / confusion / delirium.

Ø Able to work independently in a number of environments as an autonomous practitioner.
Ø Provide comprehensive specialist mental health assessment of a referred patients mental health needs, formulating and implementing nursing care plans and goals which will reflect the needs of the patients over a 24-hour period.
Ø Regularly review caseload management with their clinical supervisor.
Ø Maintain accurate records about the care and treatment offered.
Ø Ensure that clinical, supportive, and therapeutic interventions protect the rights of the patient and are in accordance with evidence-based practice and Trust policies.
Ø Adhere to confidentiality principles and maintain the rights of individual patients.
Ø Contribute personal and professional skills, therapeutic use of self and those of other nurses under their supervision, to ensure the development of the Mental Health Team
Ø Identify and manage risks in variety of settings.
Ø Offering support and advice to families and carers and acting as an advocate for clients, where appropriate.
Ø Contribute to managing the team caseload by the effective use of time and resources.
Ø Provide constructive supervision in clinical managerial and educational needs in relation to staff development and Risk Management.
Ø Influence/make and implement changes to clinical practice, locally and trust wide.
Ø Participate in regular supervisions, individual appraisal performance reviews and the construction of personal developments plan.
You will possess excellent clinical triaging and assessment skills using a Biopsychosocial model and be able to work effectively with service users and their relevant others to formulate collaborative plans prior to their discharge from Sheffield Teaching Hospitals.
Knowledge and understanding of PDR and supervision processes are also essential requirements for the post together with a commitment to the appraiser and supervisor role. We promote regular supervision and team discussion / debrief as part of our working culture.
Additionally, you will need to possess excellent communication skills, be motivated, able to work independently, and as part of a multidisciplinary team and with other partnership agencies. You should have a positive approach to meeting the needs of our service users and be able to demonstrate continued professional development
