Description

The Specialist Occupational Therapist (OT) functions as part of the Multi-Disciplinary Team (MDT) to provide a seamless service to a group of patients.
The OT will develop, implement, and evaluate the Merton Enhanced Home-Based Rehabilitation Team (MEHBRT) , ensuring that patients receive the highest standard of timely care within the trust
The Specialist Occupational Therapist (OT) functions as part of the Multi-Disciplinary Team (MDT) to provide a seamless service to a group of patients.
The OT will develop, implement, and evaluate the Merton Enhanced Home-Based Rehabilitation Team (MEHBRT), ensuring that patients receive the highest standard of timely care within the trust.
The OT will clinically assess referrals from hospitals for patients who are ready to be discharged but have ongoing health or social care needs.
The OT will provide local clinical leadership and advice to Physiotherapists, Rehabilitation Support Worker and the wider MDT within the CSU.
The OT will be responsible for the line management and supervision of junior staff within the service.
The OT will manage their own caseload, including complex cases and request support from Senior members if needed.
The OT will lead/support the team in achieving key performance indicators (KPIs) and maintaining the quality of the service.As part of an interdisciplinary team within MEHBRT , the OT will contribute to developing strong working relationships with statutory, voluntary, and independent organisations.
The OT will provide competent and compassionate care to all patients with physical and cognitive difficulties within community and/or care home settings
