The Intermediate Care Team aims to provide an excellent standard of care which normally lasts no more than two weeks. This means we give an assessment, treatment, and monitoring to support people with complex, difficult, and reoccurring health care needs. Our patients might be at home, in hospital, or in A&E. We give our patients a care plan that’s specific to them. We know we’ve done our job well when people can live in their own homes or independently after they’ve been ill or injured.
You will be working in a multidisciplinary team that includes consultants, nurses, physiotherapists, rehabilitation technicians and Administrative Staff.
The teams operate 7 days a week ensuring the delivery of the service can react to the patients’ needs as and when required. The shifts for the service are between 8:30am – 18:00pm across , including weekend and bank holiday working as part of the rota.

You will need to be adaptable and resourceful. You’ll need to be well organised, confident, and able to cope in diverse situations. Leadership and management skills are also important, as with all roles, communication skills, problem solving, and good judgement are crucial.

Join us and become part of Team NHFT.
NHFT is an integrated primary care and mental health Trust, providing physical, mental health and specialty services in both hospital settings and out in the community. Because we put the person at the centre of all we do, we focus on delivering care that is as easy to access as possible. This means many of our services can be provided at home, work or in schools. We also provide health services to various prisons and detention centres in Bedfordshire and Cambridgeshire.
