Posting Date 11/20/2025 966 E Hospitality Ln, San Bernardino, California, 92408-2818, United States of America The Patient Education Specialist is primarily responsible for facilitating and performing 1:1 (and sometimes group-based) Home Modalities education to in-center patients, with a special focus on patients within their first 90 days of dialysis (i.e. incident patients). Other responsibilities include mentoring, teaching, supporting, and working closely with the Home Ambassadors from each of the in-center facilities in the region, as well as providing meaningful and regular Home Modalities education to teammates in the region. The Patient Education Specialist is also responsible for facilitating communication and follow up between in-center and Home clinics, ensuring that patients have a timely and smooth transition to Home.
