Description

An opportunity has arisen within the CNWL NHS Foundation Trust, Community Health Services, for a Specialist Physiotherapist to join our Staying Steady, falls prevention service. Due to the expanding and innovating project work, currently under development, we are further resourcing our team to carry out this exciting new work. Based in Bletchley, Milton Keynes, Staying Steady is a multi-professional team who undertake multifactorial risk assessments, provide enabling solutions using equipment prescription, advice, signposting and other treatment interventions to reduce falls risks for adults in Milton Keynes.

This post will provide an opportunity to further develop your assessment and treatment skills for people who have fallen or are at risk of falling. You will be working alongside experienced and specialist Physiotherapists, Occupational Therapists, Nurses, Rehabilitation Support Workers and Consultant, to provide coordinated assessment and goal orientated intervention to service users.

  • To provide advanced assessment of patients who are at risk of falling or who have fallen, to develop and deliver individualised treatment programmes.
  • To undertake telephone triage, multi-factorial assessments, equipment and other therapeutic input as part of the Multi-professional Team.
  • To hold responsibility for own caseload and be responsible for a defined area of service or a particular patient type, working without direct supervision. Supervision takes the form of regular formal training and clinical reasoning sessions, peer review and case conferences. Access to advice and support from a Senior HCP is available when required, clinical work is not regularly evaluated.
  • To undertake all aspects of clinical duties as an autonomous practitioner.
  • To undertake evidence-based audit and research projects to further own and team’s clinical practice. Make recommendations to Clinical/Lead Manager of service for changes to practice by the team. May lead the implementation of specific changes to practice or contribute to service protocols.

The continuing development of the academic learning and vocational skills of the therapy team is imperative. Your participation in staff supervision, training and completion of competencies is key to the provision of the ongoing standard of care we expect at CNWL.

  • To be professionally and legally responsible and accountable for all aspects of your own work including the management of patients in your care and that of students and assistants under your direct supervision. To ensure a high standard of clinical care for the patients under your management.
  • To undertake the comprehensive assessment of patients, including those with a complex presentation/multi-pathologies; use advanced clinical reasoning skills and manual assessment techniques to provide an accurate assessment/diagnosis of their condition.
  • To formulate, deliver and evaluate individual treatment programmes based on a sound knowledge of evidence based practice and treatment options using clinical assessment, reasoning skills and knowledge of treatment skills.
  • To take delegated responsibility from the Falls Prevention Coordinator for managing patients with particular conditions and be responsible for providing specialist assessment and treatment plans for patients with these conditions.
  • Formulate accurate prognoses and recommend best course of intervention, developing comprehensive discharge plans.
  • Assess patient understanding of treatment proposals, gain valid informed consent, and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.
  • Use a range of verbal and non-verbal communication tools to communicate effectively with patients to progress rehabilitation and treatment programmes. This will include patients who may have difficulties in understanding or communicating.
  • To manage clinical risk within own caseload and that of students under direct clinical supervision.
  • To represent the service and/or individual patients at the multidisciplinary team meetings, to ensure the delivery of a coordinated multidisciplinary service, and integrate treatment into the treatment programme. This will include discussion of patient care, patient progress, and involvement in discharge planning.
  • To be responsible for the safe and competent use of all appropriate equipment.
