Description Job Description Summary: The Beyond Faith Health & Wellness Speech Language Pathologist is responsible for ensuring that quality Speech therapy care is coordinated, managed, and delivered appropriately in a cost-effective and financially responsible manner. Essential Job Functions/Responsibilities: A. Provides Speech therapy services and manages patients’ Speech therapy plans of care in an outpatient setting. Related duties include, but are not limited to: Conducting complete Speech therapy assessment and evaluation of each patient, employing appropriate tests and measures in the examination process; Establishing an individualized Speech Therapy plan of care for each patient, including consultation with the appropriate medical provider to obtain required authorizations and concurrence with the plan of care; Providing direct patient therapy services in accordance with prevailing professional practice standards (according to the American Speech-Language-Hearing Association); Adherence to all confidentiality and privacy policies, in accordance with organizational policies and federal regulations. B. Consults with other healthcare team members and the community, as appropriate. Identifies patient and family/caregiver needs for other health services and refers as necessary; Provides Speech therapy consultation and screening to families/caregivers and other customers as appropriate; Participates in discharge planning for patient and communicates to appropriate healthcare team members, patient and family/caregivers; attends team conferences and staff meetings as appropriate/requested; Participates in community education, marketing, public relations and promotional events, as requested, and with appropriate approval. C. Prepares and submits required, accurate and timely documentation in a facility- and regulatory-compliant format, to include, but not limited to: Consults with referral sources, including plan of care for MD authorization/certification review date; Evaluation and ST plans of care; Treatment records, including session notes and progress reports; Additional treatment and billing records and charges, according to organizational procedures and systems; Additional documentation as requested for audit or other review purposes. D. Other duties, as assigned, including but not limited to: Participates in program development as requested; Participates in performance improvement initiatives and projects as requested; Contributes to maintenance of safe and clean clinical environment, including participation in equipment maintenance, monitoring for regular inspection and reporting identified service or repair needs. Requirements Position Qualifications: Entry-level education from an appropriately accredited Speech therapy academic program Active license in good standing for the state in which the clinical agency is located. At least 1 year of clinical practice as a Speech Language Pathologist, preferred. Demonstration of good verbal and written communication and organizational skills. Possesses and maintains current CPR certification for healthcare providers, obtained through acceptable CPR certification provider
