Description

We are seeking a compassionate and motivated PRN Speech Language Pathologist (SLP) to join our growing Home Health team. This flexible role is ideal for clinicians who value autonomy, meaningful one-on-one patient care, and the ability to truly impact recovery in a home-based setting. As a Home Health SLP, you will provide skilled therapy services to patients in their homes, helping them regain communication abilities, improve swallowing safety, and enhance overall quality of life. ---------------------------------------- WHAT YOU’LL DO * Evaluate and treat patients with speech, language, cognitive-communication, voice, and swallowing disorders in the home setting * Develop individualized care plans based on comprehensive assessments and physician collaboration * Provide dysphagia management, including swallowing strategies and safe diet recommendations * Implement therapy techniques to improve articulation, fluency, cognition, and functional communication * Train patients and caregivers on communication strategies and safe swallowing techniques * Document evaluations, progress notes, and discharge summaries in compliance with regulatory and agency standards * Collaborate with interdisciplinary team members to ensure coordinated, patient-centered care * Adjust treatment plans based on patient progress and home environment needs ---------------------------------------- WHAT WE’RE LOOKING FOR * Current Speech Language Pathologist (SLP) license in the state of Nevada * Certificate of Clinical Competence (CCC-SLP) * Previous home health experience strongly preferred, but not required * Strong clinical judgment and ability to work independently * Excellent communication and patient education skills * Reliable transportation for in-home visits Requirements * Valid Speech Language Pathologist License * 1 year of rehab experience preferred, not required * CPR certification * Valid Driver’s License * Valid Auto Insurance * Long Term Care experience preferred * Vital-Stim certification preferred * HomeCare HomeBase experience preferred Physical Demands * Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods. * Must be able to lift, carry, pull, or push up to 100 lbs..
