Description About the Role Touchstone Home Health is seeking a Speech Language Pathologist (SLP) to join our San Antonio team—where clinical excellence meets purpose-driven care. Key Responsibilities Provide speech therapy services in accordance with physician orders and individualized care plans Conduct comprehensive evaluations of communication and swallowing needs Develop and adjust treatment plans to support patient progress and safety Educate patients, families, and caregivers on home-based strategies and exercises Complete timely and accurate documentation, including OASIS assessments and care updates Collaborate with physicians and interdisciplinary teams to coordinate high-quality care Requirements What We’re Looking For Master’s or Doctoral degree in Speech Language Pathology Current SLP license in the state of Texas (or eligibility) Strong communication skills and the ability to connect with diverse patients and families Adaptability in dynamic, home-based care environments Organized, self-directed approach to managing schedule and documentation Willingness to travel between patient homes throughout the San Antonio area Why Join Us At Touchstone Home Health, you’re not just stepping into a role—you’re stepping into a mission: to bring out the courage in others.
