Description About the Role Maryland Physicians Care (MPC) is seeking a dynamic Senior Director of Population Health to provide strategic leadership and oversight of our Population Health team. This position collaborates across departments to support broader population health efforts, recognizing that many related activities are shared across the organization. This role is pivotal in advancing initiatives that improve the health and well-being of MPC’s membership. As a key leader, you will: · Drive Population Health Strategy: Develop and implement integrated strategies that address physical and behavioral health needs. · Advance Health Equity: Incorporate social determinants of health and promote equity across race, ethnicity, language, disability, sexual orientation, and gender identity. · Lead Data-Informed Initiatives: Utilize analytics, risk modeling, and community assessments to guide interventions and improve outcomes. · Collaborate Across Teams: Partner with internal leaders and external organizations to align population health initiatives with MPC’s strategic priorities. · Champion MPC’s Vision: Communicate and embed the goals and values of our comprehensive population health model across the organization. · Build Community Partnerships: Foster sustainable relationships with public health agencies, social services, local governments, health systems, and community-based organizations. · Provide Expertise: Serve as a subject matter expert for new and existing business opportunities related to population health. What You’ll Do: Establishes, informs, monitors, and accomplishes the health plan’s integrated population community health strategy, in conjunction with the CMO. Partners with other organizational departments and teams to coordinate activities to accomplish the integrated population health strategy. Oversee a population health team to measure the needs of the population and implement interventions to address the determined needs. In conjunction with the CMO, develops and implements the MPC Social Determinants of Health plan. Champions integrating and articulating the goals and values of the health plan’s comprehensive population health model to leadership, associates, and key stakeholders. Employs and implements core concepts of population health improvement such as social determinants of health, health service research, epidemiology, and statistics to guide the health plan’s strategic vision. Builds, preserves, and prioritizes collaborative, sustainable relationships with public health, social services, local government agencies, health systems, and community-based organizations. Supports compliance with all contractual, regulatory and accreditation requirements for lines of business. Assures consistency of population health program implementation and staffing to ensure value and achievement of ROI. Requirements Knowledge and Skills: Excellent communication and interpersonal skills to include the ability to negotiate and resolve conflicts and build teams. Broad knowledge of modern health care administration practices and principles within a Medicaid managed care environment. Effective analytical ability to develop and analyze options, recommend solutions to, and solve complex problems and issues. Demonstrated effective administrative leadership. This position has significant field work involved, with frequent travel to partner organizations, including state and local health departments, and community-based organizations. This role requires approximately 15–20% travel. Education and Work Experience: Minimum bachelor’s degree in healthcare related field; Public Health, Social Work, Health Services Research, Health Policy, Epidemiology, etc. Strongly preferred master’s degree. Seven (7) or more years of experience in community health improvement, public health or closely related relevant field such as population health, service coordination, case, or care management within a healthcare environment, preferably Medicaid/Medicare managed care organization. 5 or more years of experience using claims, EHR data and qualitative data to identify gaps in care and opportunities to improve member outcomes. Valid MD Driver’s License.
