To assist with the successful onboarding, training, and retention of paraprofessional staff. To provide personal care, meal preparation, and light housekeeping assistance to clients. Home health aides are selected on the basis of such factors as empathy, ability to read, write and carry out directions, and maturity and ability to deal effectively with the demands of the job.

QUALIFICATIONS

  • Individual must hold a valid Certificate of Completion as a Home Health Aide issued by NYS Department of Health.
  • Must be at least 18 years of age or older.
  • Must have a valid NYS Driver’s License and proof of auto insurance.

PHYSICAL REQUIREMENTS

Must be able to perform the physical activities associated with the position, including bending, stretching, reaching, kneeling, climbing stairs, and moving/lifting up to 40 lbs. repeatedly; pushing, pulling, and transferring up to 100 lbs.; and completing written documentation and record-keeping.

SENSORY ABILITIES SPECIFICALLY REQUIRED:

Vision: Visual acuity required to read plan of care, order, etc.
Hearing: Hearing acuity require to perform all aspects of the job.
Touch/Feeling: Has sufficient hand strength and dexterity to administer the care plan duties.

Responsibilities
  • Mentor and support paraprofessional staff by modeling ICARE values, providing constructive feedback, and assisting with training and skill development.
  • Provide hands-on personal care to patients, including bathing, mobility assistance, transfers, and range-of-motion exercises, following care plans and safety procedures.
  • Assist with meal preparation, light housekeeping, and errands to maintain a safe, clean, and supportive home environment.
  • Observe and report changes in patient condition to the Case Manager.
  • Respond to emergencies according to agency procedures.
  • Utilize agency-issued technology for documentation and communication.
  • Maintain reliability, adherence to the dress code, and readiness to work within assigned availability and service areas.
  • Achieve performance goals (KPIs) and complete required in-service trainings.
  • Perform other duties as assigned to support patient care.
  • Other duties as assigned.
