Description
  Responsibilities: * Maintains and enhances the organization's Department of Pharmacy by involvement in the selection and procurement of medications, storage of medications, medication order entry, preparation and labeling of medications, dispensing of medications, and medication process quality improvement activities. * Responsible for all the duties intended to properly identify each patient.  * Prepares and dispenses medication orders per physician request according to established policies, procedures, and protocols. * Interprets medication orders accurately and maintains complete patient medication profiles. * Compounds and accurately dispenses pharmaceuticals, including sterile products. * Issues controlled substances to patient care areas and maintains records as required by law. * Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose. * Participates in the pharmacy pharmacokinetic dosing service and drug dosing service per organizational protocol. * Reviews/interprets culture and sensitivity data for antibiotic appropriateness and recommends changes as needed. * Participates in the pharmacy clinical program which includes medication profile review and the ability to accurately read, extract, and interpret patient chart information. * Documents clinical activities and interventions accurately and completely. * Detects and reports suspected adverse drug events and medication errors accurately and in a timely fashion. * Sustains the formulary by minimizing non-formulary procurements by utilizing therapeutic substitution protocols and promoting rational drug therapy selection. * Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost effective therapeutic alternatives to medical staff as needed. Provides accurate, adequate, and timely drug information to the professional staff.
