In this role, you’ll assess resident health status, develop and update care plans, communicate with staff and families, and ensure compliance with all regulatory standards. You’ll foster a safe, compassionate, and engaging environment while supporting the professional growth of the care team. What You’ll Do * Assess residents’ health upon admission and ongoing changes in condition, and develop or update individualized care plans * Communicate resident needs and changes to the Director, Health and Wellness, staff, and families * Participate in hands-on care and perform specific nursing tasks per residents’ care plans * Train and support care staff to meet continuing education and competency requirements * Transcribe and review physician orders; monitor medication administration and perform MAR reviews * Ensure compliance with state, federal, and local regulations, including HIPAA, OSHA, and Bloodborne Pathogen Guidelines * Participate in Continuous Quality Improvement (CQI) initiatives and report issues to leadership * Work safely and model safe practices for staff; correct unsafe actions as needed * Foster teamwork, show initiative, and positively impact coworkers, residents, and visitors * Report any elder maltreatment or abuse in accordance with policy * Perform other duties as assigned What We’re Looking For * Associate’s or Bachelor’s Degree in Nursing preferred * Valid State Nursing License (RN or LPN/LVN); required certifications as mandated by state * Minimum 1 year of nursing or caregiving experience, preferably in Assisted Living or Memory Care * Experience with Microsoft Word and Excel; familiarity with human resource rules a plus * Strong clinical assessment skills, conflict resolution, and problem-solving abilities * Excellent written, verbal, and interpersonal communication skills * Compassion for and desire to work with the elderly * Self-motivated, organized, detail-oriented, and able to work independently * Pleasant, professional, and personable demeanor Join Milestone to provide exceptional clinical care, support a collaborative team, and positively impact residents’ lives every day.
