REQUIREMENTS:

• Degree in Nursing and/or Advanced Diploma in Palliative Care or other specialty preferred
• Minimum 3 years’ experience working in working in acute hospital setting and/or community hospital setting.
• Registered Nurse with the Singapore Nursing Board

• Deliver nursing care, including wound care, medication management, and continence care, while ensuring safety and infection control.
• Conduct biopsychosocial assessments and create personalized care plans in collaboration with clients, families, and caregivers.
• Conduct caregiver training and promote self-management through health education and motivational techniques.
• Work with healthcare teams for integrated care, transitions, and referrals to other services.
• Initiate emergency protocols when needed and escalate patient concerns to supervisors.
• Maintain accurate patient records, ensure confidentiality, and adhere to healthcare policies and financial procedures.
• Facilitate ACP discussions and provide bereavement support to families and caregivers.
• Participate in audits, process improvements, committee work, and rotating shift duties, including weekends/public holidays.
