Degree in Nursing and/or Advanced Diploma in Palliative Care or other specialty preferred
Minimum 3 years’ experience working in working in acute hospital setting and/or community hospital setting.

Responsibilities
Deliver nursing care, including wound care, medication management, and continence care, while ensuring safety and infection control.
Conduct biopsychosocial assessments and create personalized care plans in collaboration with clients, families, and caregivers.
Conduct caregiver training and promote self-management through health education and motivational techniques.
Work with healthcare teams for integrated care, transitions, and referrals to other services.
Initiate emergency protocols when needed and escalate patient concerns to supervisors.
Maintain accurate patient records, ensure confidentiality, and adhere to healthcare policies and financial procedures.
Facilitate ACP discussions and provide bereavement support to families and caregivers.
Participate in audits, process improvements, committee work, and rotating shift duties, including weekends/public holidays.
