JOB SUMMARY

We are seeking a dedicated and knowledgeable Staff Pharmacist to join our healthcare team. The ideal candidate will possess a strong background in clinical pharmacy, patient care, and medication management, and must be registered with the Ontario College of Pharmacists (OCP). This role involves close collaboration with healthcare professionals to ensure optimal patient outcomes through effective medication therapy management. The Staff Pharmacist will be responsible for providing comprehensive pharmaceutical care, conducting patient assessments, and maintaining compliance with all applicable regulatory standards.

QUALIFICATIONS

• Pharmacy degree from an accredited institution.
• Licensed and in good standing with the Ontario College of Pharmacists (OCP).
• Strong understanding of medical terminology, pharmacology, physiology, and anatomy.
• Familiarity with compounding techniques is essential.
• Proficient in using Kroll pharmacy software.
• Excellent communication and patient service skills.
• Ability to work collaboratively within a multidisciplinary healthcare team.

• Deliver comprehensive pharmaceutical care, including medication therapy management and patient education.
• Conduct patient assessments to determine appropriate medication regimens based on clinical findings.
• Review and interpret physician prescriptions for accuracy, appropriateness, and therapeutic efficacy.
• Compound medications when necessary, following aseptic techniques and safety protocols.
• Administer immunizations and educate patients on vaccine-preventable diseases.
• Monitor patients for potential drug interactions, adverse effects, and therapeutic outcomes.
• Maintain accurate records using pharmacy software (Kroll) for patient documentation and medication tracking.
• Manage inventory efficiently, ensuring adequate stock of medications and supplies.
• Ensure compliance with HIPAA regulations and maintain patient confidentiality.
• Participate in pain management and other clinical initiatives to support medical treatment plans.
