FTE: 1.000000 Shift: Shift 1 Job Summary: This position focuses on health care for individuals, families, groups, communities and populations which occurs in primary and specialty care outpatient clinics. Addresses in partnership and collaboration with other health care professionals the patient's wellness, acute illness, chronic disease, disability, and end -of-life needs. Responsible for patient advocacy, coordination of nursing and other health services, implementation of nursing services and continuity of care. Provides patient education when needed. JOB REQUIREMENTS/PREFERRENCES: Graduate of an accredited school of nursing required, and current unrestricted Wisconsin RN license required. None required; at least one year medical/surgical nursing preferred. CPR certification required. Excellent communication and organizational skills required.
