Collaborate with transdisciplinary team in a variety of settings. WHAT YOU WILL DO: * Provide personal care and assist with ADL's according to the POC. * Provide safe environment for patients. * Safely ambulate/transfer patient as appropriate. * Assist with passive and active ROM exercises as directed by the nurse. * Prepare nutritional snacks as appropriate for the patient. * Under the supervision of the nurse, assist patient with pre-poured medications that are ordinarily self-administered and that are prescribed by a physician under the supervision nurse (Home Care only). * Under the direction of the nurse, perform simple wound care, skin care, and bowel care protocols according to the plan of care where appropriate. * Recognize and promptly report any changes in the mental or physical status of the patient to the nurse. * Record in clinical record, on appropriate documents, the activities provided and observations of patient at the time of visit. * Complete all charting, timesheets and mileage logs as defined by the agency policy and procedures. * Attend and actively contribute during patient care conference. * Participate in HNA discipline meetings, team meetings, staff meetings, retreats and in-services as appropriate. * Attend in-services to obtain 3 education hours per quarter. * Participate in the on-call rotation as scheduled (Home Care only). * Assist the Team Leader with other designated activities as necessary (scheduling, supply organization, answering office telephone, etc.) * Demonstrate positive customer-service responsiveness according to agency standards. * Flexible in meeting patient/family needs that require variable shifts and weekends. * Participate in chart review quality improvement activities and other meetings as assigned. * Participate in coordination of and communication with volunteer staff. * Provide basic care-giving education to caregiver/family members, such as turning and positioning, transfer techniques and changing linens * Maintain Transmission Based Precaution. * Complete annual competency requirements. SUCCESS CAPABILITIES: * Must be able to read and write English proficiently. * Must be state tested nursing assistant in good standing * Equivalent education/experience may substitute for minimum qualifications except when there are legal requirements, such as a license/certification/registration.
