Sub-Acute Unit Manager is a qualified Registered Professional Nurse or Licensed Practical Nurse responsible for the development and maintenance of nursing practice and standards consistent with the overall philosophy of the facility. The Sub-Acute Unit Manager represents the Nursing Department to other departments and coordinates the services provided by other departments. Sub-Acute Unit Manager, RN represents the Nursing Department to other departments and coordinates the services provided by other departments. SUB-ACUTE UNIT MANAGER ESSENTIAL JOB RESPONSIBILITIES: During your shift you may be required to perform some or all the following: Perform professional supervision, guidance, and direction to staff in both clinical and personnel issues on the floor. Establish, coordinate, and manage daily operations of resident care on the floor. This includes but not limited to showers/baths, toileting, meal, medication administration and activities. Coordinate resident care on the floor in accordance with NYS DOH regulations. Provide support to resident/families in dealing with clinical issues and challenges. Participate in Quality Assurance Performance Improvement. Attend meetings such as Morning Meeting, Clinical Meeting, Care Plan Meetings, and QAPI on time and prepared to exchange information. Communicate to other members of the Interdisciplinary Team pertinent information to assure resident/family needs are met. Perform daily rounds of all residents on the unit. Perform assessments on residents as required by the facility - Admission, Elopement, Pain, etc.). Develop plan of care for each resident. Document and communicate plan of care on Resident Information Sheets. Investigate timely Incidents and Accidents related to residents. Provide follow up as indicated. Investigate abuse allegations and report to DON/Administrator immediately. Assist in emergency and disaster situations. Continue to gain current knowledge in LTC/SNF and Sub-Acute Care regulations. 15. Collaborate with Interdisciplinary Team then revise/update the Nursing Care Plan. Demonstrate understanding and knowledge of MDS to ensure utilization of interdisciplinary rehab team process in the formulation of MDS/RAI's. Communicate with and follow recommendations made by/with MDS on observed direct nursing care, documentation, and chart audits. Utilize the nursing process in the care of designated residents: a. Assure Assessment: § Document clinical observation and assessment on all assigned residents per regulations. § Report findings of ongoing observations/assessments to the designated Nursing Supervisor. § Document effectiveness of plan of care. § Demonstrate knowledge of basic test/lab collection values. b. Implementation: § Deliver direct resident care consistent with the policies and procedures of the Facility. § Demonstrate clinical skills necessary to deliver direct resident care. § Administer medications and treatments in accordance with policy and procedure. § Deliver and direct care consistent with Resident’s Bill of Rights. 19. Respond appropriately to patient needs: a. Identify patient concerns and needs then respond in timely manner. If appropriate, consult with other members of the health care team. b. Monitor, observe and document vital signs, treatments administered and patients’ responses. c. Demonstrate knowledge of equipment and procedures. 20. Ensure the safety of all residents in accordance with Facility & Nursing policy and procedure: a. Prepare resident assignments for Certified Nursing Assistants, considering the category of needs of the patient and competency of personnel. b. Communicate ongoing resident, family and facility plan/concerns/information to staff. c. Maintain equipment on unit and reports malfunctioning promptly. d. Adhere to fire plan, disaster plan and other safety plans established. 21. Demonstrate effective interpersonal skills necessary to ensure appropriate quality patient care: a. Interact with residents, families, and colleagues in a courteous and professional manner. b. Always maintain resident privacy and confidentiality. c. Demonstrate an awareness of patients’ rights issues as related to patient care. 22. Organize, oversee and supervise assigned unit activities and staff assignments. a. Provide a Nursing Assistant assignment for each CNA on his/her team. Communicate to CNA pertinent resident care needs. b. Monitor and evaluate the flow of activity and delivery of patient care provided by the CNA’s (turning and positioning, mealtimes, bathing, etc.). c. Assure CNA documentation records (ADL’s BM, Meal intake, etc.) are completed. Communicate to the Interdisciplinary Team members any pertinent resident findings. d. Assist in providing educational needs of staff. e. Utilize progressive discipline according to Facility policy and procedures. 23. Recognize job relationships and demonstrates a willingness to function within the Nursing Home Structure: a. Utilizes appropriate lines of authority and methods of communication in problem solving. b. Refers problems to ADON/ DON as necessary. 24. Demonstrate accountability in professional practice development: a. Attend scheduled facility in-service programs. b. Maintain CPR/IV course up to date. c. Meet annual mandatory in-service requirements. d. Promote Quality Assurance audits as directed. e. Participate in Continuous Quality Assurance programs. 25. Demonstrate accountability in practice to residents, family, colleagues, and the facility: a. Adhere to established professional nursing dress code. b. Is punctual and/or provides proper notification for lateness or absenteeism. c. Utilize benefit time appropriately. d. Utilize facility resources in an efficient, cost-efficient manner. e. Demonstrate an understanding of department priorities related to patient needs. f. When necessary, work a flexible schedule to provide continuity of care for all patients. g. Assist other staff members in completion of their duties. h. Orient new Nursing personnel as assigned. Scheduled to be “on-call” on a regular basis; as needed. Participate in interdisciplinary performance and quality improvement process Maintain current knowledge with CMS, state and federal regulatory agencies and public health guidelines 29. Ensure staff adheres to safety and infection control guidelines. In-service staff as needed in relation to resident care. 31. Ensure all HIPAA requirements are adhered to. 32. Follow all facility policies and procedures. 33. Perform other related duties as required and assigned. ADA Requirements: Candidate will have the physical ability to perform job related duties, which may require lifting, bending, stooping, stretching, pulling, and pushing up to 50 lbs. Requirements Education: a. Must be a graduate of an approved school of nursing. b. Current licensure in New York State as a Registered Professional Nurse 2. Work Experience: a. Minimum three (3) years’ experience in a supervisory capacity either in a hospital, long term care facility, or other related medical facility, training in rehabilitative and restorative nursing practice. b. Long term care experience, preferred. c. Sub-acute care experience, highly preferred. 3. Job Knowledge/Requirements: a. Must have a sincere desire to work with the elderly, chronically ill, and disabled. b. Must have a thorough knowledge of medical, social, and psychological needs of residents. c. Must utilize good judgment in emergency situations. d. Must possess a spirit of cooperation and enthusiasm in order to create an atmosphere conducive to rehabilitation and growth. e. Must be able to communicate well verbally with residents, families, and staff. f. Must be able to handle emergencies with calm and intelligent response. g.
