About the Role: Be part of Canberra Health Services’ Decision Support Unit, where data, clinical insight and funding sustainability come together. As a Sub and Non-Acute Patient (SNAP) Officer, you’ll work closely with multidisciplinary clinical teams to accurately assign and update patient care types using AN‑SNAP classifications. Your work ensures high‑quality data, timely funding optimisation, and better planning, research and patient outcomes. You’ll coordinate mandatory sub‑acute and non‑acute assessments and play a key role in supporting patient recovery, independence and smooth transitions from acute care to discharge. This is a multi-classified position with possibility of appointment at either HP3 or RN2. There is only one position available. Please Note: • A merit pool will be established from this recruitment to fill any SNAP Officer HP3 or RN2 permanent and/or temporary vacancies arising over the next 12 months (of a duration up to 12 months with a possibility of extension or permanency) in a full time or part time capacity. • SNAP Officers can be asked to work across TCH, UCH and NCH sites. • Please include the names of two professional referees. • Selection for this position and the merit pool may be based on written applications and referee reports.
