Job Details Job Location: Barron Center - Cincinnati, OH 45203 Position Type: Full Time Education Level: High School Salary Range: $23.00 Hourly Job Shift: Day Job Category: Nonprofit - Social Services Position Overview Shelter House is a community of residents, staff and volunteers working together to provide basic human services for the men and women of the Greater Cincinnati area who are experiencing homelessness. The Substance Abuse Care Coordinator is responsible for coordinating care to Medically Assisted Treatment (MAT) clients and other appropriate levels of care based on AOD health assessment by developing, monitoring and evaluating interdisciplinary medical and behavioral health care plan. Key Responsibilities & Expectations Orients and educates patients about MAT process. Monitors the delivery of care with the interdisciplinary team. Evaluates the outcomes of patient care. Facilitates CD educational groups Follow best practices and complete other assignments as directed by the supervisor. Meet with client’s after assessment is completed by recovery counselor and discuss appropriate referrals and services based on assessment Enroll clients in Medicaid and have proper ID documents Provide NARCAN training to new employees Complete AOD assessments as required Workdays are Tuesday - Saturday (non-negotiable). Must have clean driving record. (A driving history free from major violations, such as DUI/DWI, reckless driving, license suspensions, or excessive moving violations.) Qualifications Qualifications & Requirements Bachelor’s degree required Must have CDCA (Required and must be active) Licensed Social Worker (LSW) preferred. Knowledge and experience with homelessness, mental health and/or substance abuse and community resources. Must pass criminal background check. Since the counselors are responsible for driving clients in their personal vehicle, they need to have the following: Valid driver’s license with good driving record, daily access to private insured vehicle during work hours and the ability to be approved by Shelter House insurer. Skills & Abilities Ability to function effectively in a high energy environment. Ability to work independently and work flexible hours. Excellent written and verbal communication skills. Possess self-initiative and demonstrated attention to detail. Computer literacy with MS Office Suite. Experience with VESTA is a plus.
