ABOUT THE ROLE:

As a Support Coordinator, you will work closely with participants, their families, and providers to ensure supports are delivered effectively and in line with individual goals. You will help participants understand their NDIS plan, build their confidence in managing it, and ensure they get the best outcomes possible.

WHAT YOU’LL DO:

-Assist participants in understanding and implementing their NDIS plans.
-Connect participants with appropriate service providers and community supports.
-Coordinate supports and monitor service delivery to ensure quality outcomes.
-Build participants’ skills and independence to self-manage their plans where possible.
-Maintain accurate documentation and compliance with NDIS guidelines.
-Advocate for participants’ rights and address any barriers to their supports.
