Description KFI is seeking a dynamic and dedicated individual to join our Leadership team in the role of Support Coordinator. KFI distinguishes itself as an award-winning agency known nationally for providing forward-thinking person-centered services for adults with disabilities. Our services focus on supporting individuals to live their best life on their own terms including community involvement, meaningful employment and meaningful choices in day to day life. The Support Coordinator is responsible to continually discover people’s wants, needs and desires and identify support strategies. They manage Direct Support Professionals who provide Home, Community and Work supports and coordinate a system of paid and natural supports that will enable people receiving support to live their best life. They also promote principles of SRV (Social Role Valorization) and assist people to advocate for themselves. The support Coordinator acts as the primary liaison with families, guardians, and Community Case Managers. This position involves regular interaction with people supported, KFI staff, family members, Community Case Managers, members of the community, and DHHS personnel. In addition to overseeing services for 10-15 people, Support Coordinators are the direct supervisor for Direct Support Professionals, proving individual and team guidance. They also work with a small number of families who are in a Shared Living arrangement.
