The Role
We’re seeking a passionate and organised Support Coordinator to join our team. In this role, you’ll assist NDIS participants in understanding and implementing their plans, connecting with supports, and building capacity to live independently.

Key Responsibilities

  • Work with participants to identify needs, goals, and suitable supports
  • Build and maintain strong relationships with participants, families, and providers
  • Monitor progress, address barriers, and adjust supports as needed
  • Maintain accurate records and complete required reporting
  • Travel to participant meetings and service visits, including some out-of-town travel

What We’re Looking For

  • Experience as a Support Coordinator, Case Manager, or in a similar role (NDIS knowledge highly valued)
  • Strong organisational skills and the ability to manage multiple participants
  • Excellent communication and interpersonal skills
  • A commitment to professional boundaries and participant choice and control
  • Ability to work independently and as part of a team

Requirements

  • Current NDIS Worker Screening Check (or willingness to obtain)
  • Current NDIS Module (or willingness to obtain)
  • Current WWC (or willingness to obtain)
  • Current driver’s licence and reliable vehicle
  • First Aid & CPR (or willingness to obtain)
  • Willingness to travel out of town as needed

Pay: $40.00 – $48.00 per hour
