OVERVIEW:

Community Living Port Colborne-Wainfleet (CLPCW) is a charitable and progressive organization employing over 150 staff who deliver support to over 200 people with intellectual disabilities. Our organization has a proud 60-year history of providing developmental services and inclusive programs within the communities we serve.

QUALIFICATIONS:

  • A diploma in Developmental Services, Social Services or related field
  • 2 years’ experience supporting people with physical and intellectual disabilities, Autism, mental health, and aggressive and challenging behaviour preferred.
  • Willingness to work with challenging behaviours

COMPETENCIES, SKILLS AND ABILITIES:

  • Ability and willingness to follow and implement supports from a Behaviour Support Plan/ Behavioural Support Strategies.
  • Experience in supporting people in their daily activities
  • Knowledge and understanding of how to facilitate and support Person Centered Planning.
  • Effective communication and interpersonal skills.
  • Excellent organizational skills and ability to multi-task.
  • Ability to take initiative.
  • Comfortable with computer applications an asset.

Responsibilities
  • Daily Support: Support each person according to their ISPs and daily schedule considering the value of flexibility. Assist and support the person to make informed choices and decisions with consideration of responsibility as well as risks
  • Behavioral Support Plan/Behaviour Support Strategies (BSP or BSS): Consistently implement the BSP or BSS, while ensuring that least restrictive interventions are always followed prior to more intrusive interventions. Ensure that all applicable documentation is completed. To demonstrate knowledge of behavioural intervention strategies in a consistent and professional way.
  • Activities: Introduce the person to a variety of meaningful activities so that he/she can make an informed choice/decision as to what he/she wants to continue/discontinue, utilizing community resources and systems i.e. churches in the denomination of the person, transportation, social activities, etc. Support the person in achieving positive and fulfilling roles within his/her community.
  • Standard of Needs and Care: Ensure the required support is provided to the person in all areas of his/her needs that include but are not exclusive to: medical, personal, physical, financial, psychiatric, equipment, hygienic, social, recreational, emotional etc. Ensure they are met on a day to day basis according to the person’s needs, wants and outcomes.
  • Medication: Support the person with proper administration and recording of medications and treatments, updating all prescription and non-prescription medication as required. Ensure medication/MAR sheets are accurate and up to date when you sign, ensure Medication Procedures are followed.
  • Communication: To support a person to communicate in different ways so they can express themselves effectively (to ensure the person has a voice). Also, communication with your coworkers to ensure that the continuity of care to each person supported is provided.
