JOB SUMMARY

We are seeking a compassionate and dedicated Support Worker to join our team. The ideal candidate will provide essential assistance to individuals in various care settings, including group homes, assisted living facilities, and private residences. This role involves supporting clients with daily living activities, ensuring their safety and well-being, and fostering a positive environment that promotes independence and dignity.

REQUIREMENTS

  • Experience in home health, hospice care, or long-term care settings is preferred.
  • Knowledge of first aid and CPR is essential; certification is a plus.
  • Familiarity with patient observation techniques and monitoring vital signs.
  • Ability to perform heavy lifting as required for client mobility support.
  • Strong communication skills and the ability to work collaboratively with a diverse team.
  • Previous experience working with individuals with disabilities or in memory care settings is highly desirable.
  • A background in social work or nursing can be beneficial but is not mandatory. Responsibilities
  • Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and meal preparation.
  • Provide patient care and support to individuals with developmental disabilities, Alzheimer’s, dementia, and autism.
  • Monitor patient vital signs and report any changes in condition to the appropriate healthcare professionals.
  • Maintain a clean and safe environment for clients by performing light housekeeping duties.
  • Assist in the administration of medications as per care plans while adhering to HIPAA regulations.
  • Engage clients in social activities and encourage participation in community events to enhance their quality of life.
  • Utilize equipment such as Hoyer lifts for safe patient transfers when necessary.
  • Document care provided and maintain accurate records of client progress and needs.
