Support Worker Job Description
Duties:
- Provide direct care and support to individuals with various needs.
- Assist with daily living activities, such as meal preparation, personal hygiene, and medication administration.
- Implement behavior management strategies to promote positive behaviours and ensure the safety of individuals.
- Monitor and record vital signs of individuals as directed by medical staff.
- Collaborate with case managers and healthcare professionals to develop and implement individualized care plans.
- Maintain accurate documentation of services provided and individual progress.
- Advocate for resident rights and ensure a safe and supportive environment.
- Maintain confidentiality in accordance with HIPAA regulations.
- Drive individuals to appointments, outings, and community activities as needed.
Qualifications:
- Previous experience working in a similar role or relevant healthcare field is preferred.
- Knowledge of medical terminology and procedures is an asset.
- Ability to effectively communicate with individuals, their families, and healthcare professionals.
- Strong organisational skills and attention to detail.
- Ability to handle challenging behaviors and provide behavior management support.
- Valid driver’s license with a clean driving record.
