Purpose of the Position

•  The position provides structured, participant-centred support to individuals and families experiencing complex personal, social and family challenges. The successful applicant will independently manage a caseload for NDIS participants, directly assessing their needs, conducting risk assessments, prepare and review support plans, coordinate referrals, and assist participants to access services such as but not limited to: health, welfare, housing, education, employment and other community services. The position requires regular collaboration with families, community organisations, government agencies and multidisciplinary professionals.
