Responsibilities

PURPOSE OF THE POST:

To provide specialised, trauma-informed support to adults and families who have experienced domestic or sexual abuse, harm, or violence, within our accommodation and community services across Torfaen
This includes regular time as part of a rota in our Torfaen Hwb, providing trauma-informed responses to referrals, undertaking compassionate, strengths-based conversations to understand need and risk for adults and families who have experienced domestic and /or sexual abuse, harm, or violence.

SPECIFIC RESPONSIBILITIES

  1. Work collaboratively with team members to provide person-led support to a caseload of adults and/or families across all services in Torfaen
  2. Creating a safe, respectful environment where individuals feel heard, have choice, and are supported to make informed decisions about their future.
  3. Regularly assess and sensitively manage risk, working in partnership with individuals to promote safety in a way that is inclusive, strengths-based, and enabling.
  4. Support individuals to co-develop personalised support and/or safety plans, ensuring goals are meaningful, achievable, and led by the person’s own priorities and experiences.
  5. Adhere to safeguarding legislation, policies, and procedures, responding to concerns promptly and appropriately, while maintaining a trauma-informed and non-judgemental approach.
  6. Maintain accurate, timely, and confidential records that reflect the support provided, ensuring documentation is respectful, person-centred, and aligned with best practice.
  7. Work collaboratively with colleagues and the Facilities Coordinator to ensure accommodation services are safe, welcoming, and psychologically informed, including compliance with health and safety standards.
  8. Support individuals to sustain their accommodation, including understanding and upholding licence agreements or contracts, and take a person-led approach to preparing for independent living.
  9. Provide clear, compassionate signposting to internal and external services, based on the individual’s expressed needs, interests, and readiness to engage.
  10. Create regular opportunities for feedback, such as through house meetings or informal check-ins, to ensure services are continuously shaped by the voices of those we support.
  11. Contribute to audits and reporting requirements, engaging positively with improvement plans and recognising the value of reflective learning in service development.
  12. Actively participate in clinical and managerial supervision, using these spaces for reflection, learning, and maintaining emotional wellbeing.
  13. Build and maintain respectful, collaborative relationships with external agencies, championing the needs and rights of those we support.
