The Role:

As a Support Worker, your primary responsibility will be to provide support to individuals living with a mental illness and/or disability in the community, including participants with complex support needs, psychosocial disabilities, and high-risk presentations. We believe in a shared responsibility to assist people, and we're looking for proactive, resilient, and positive individuals to join our team.

Your Responsibilities:

• Assist individuals in the community living with a disability and/or mental illness.
• Provide direct support as outlined in NDIS service plans, focusing on socially inclusive activities in the community.
• Deliver direct care in the home, including social support, personal care, domestic assistance, meal prep, and other activities of daily living.
• Support participants with complex support needs, including those with personality disorders, schizophrenia, bipolar disorder, psychosocial disabilities, trauma backgrounds, and other complex presentations. You will be confident working within behaviour support strategies, risk management plans, and positive behaviour support approaches while remaining person-centred and committed to making a meaningful difference in a person's life.

Selection Criteria:

To be successful in this role, candidates must demonstrate:

1. Certificate pertinent to mental health, disability, and/or significant related experience.
2. Current First Aid & CPR ( or willingness to obtain ).
3. Current full Drivers License & Registered & Insured Vehicle.
4. Willingness to travel.
5. Current Working with Children's Check.
6. NDIS Workers Screening Check.
7. Completed all NDIS Mandatory Modules.
8. Commitment to Trucare's values of Integrity, Equity, Mutual Respect, Empowerment, Inclusion, and Opportunity.
9. Previous experience providing community-based support to individuals living with disabilities and/or mental illness, including participants with complex care requirements, psychosocial disabilities, challenging behaviours, or forensic or high-risk presentations.
10. Ability to adhere to NDIS service plans and deliver support services in accordance with individual needs and preferences.
