POSITION DESCRIPTION:
The Support Worker at Lutheran Disability Services plays a vital role in enhancing the quality of life for individuals with disabilities. This position involves providing compassionate and respectful support to clients, fostering their independence and promoting inclusion within the community.
Ability to work flexibly across a 14 day/night roster, which includes shift work, weekend, public holidays, night shifts & overnight sleepovers.
Assist clients with daily living activities such as personal hygiene, meal preparation, and medication management.
Support clients in accessing community resources and services, including educational programs and recreational activities.
Support clients with their individualised care plans in collaboration with clients, families, and other professionals to meet specific needs.
Provide emotional support and encouragement to help clients achieve their personal goals.
Document client progress and report any changes in behavior or health status to the appropriate team members.
Ensure a safe environment by following health and safety protocols, reporting hazards, incidents and maintaining cleanliness.
Engage in regular training sessions to stay updated on best practices in disability support services
