Full job description

Provide person-centred support to people with disability, assisting with personal care, daily living, community access, transport, household tasks and medication assistance. Maintain accurate documentation, follow support plans and organisational policies, and promote participant choice, dignity and independence. Work collaboratively with participants, families and the SHH team to deliver safe, high-quality supports. Experience in complex disability care is desirable.
