We are looking for experienced Care Workers / Care Assistants / Carers to join our Discharge Support Service across Gateshead. If you’re ready to support your local community and make a real difference in the lives of the people we care for, we’d love to hear from you!
You will be required to liaise with other health professionals, and signpost service users and family members to the appropriate additional support, equipment and services to aid their ongoing recovery and long term care needs.

Areas covered:

• Zone 1 – Felling, Windy Nook, Leam Lane, Deckham, Old Fold, Bensham, Teams.
• Zone 2 – Wrekenton, Birtley, Harlow Green. Zone 3- Dunston, Whickham, Sunniside, Blaydon, Crawcrook, Ryton, Prudhoe.

Shift patterns:

• You will be required to work on a rota basis between 7am to 11pm, 7 days per week.
• 7am to 2:30pm, 2.30pm to 11pm.4 on 3 off shift pattern.
• Other shift patterns and non-regular bank work may be available.

Requirements:

• Right to work in the UK - Unfortunately, we are unable to offer Tier 2 sponsorship at this time
• Previous experience in delivering person-centred care.
• Full UK driving licence with access to your own vehicle. (Staff will be provided with a car in each zone for the use of delivering the service.)
