This Opportunity
We’re currently seeking a Support Worker to join our Homelessness Services team at Accordwest. This is a dynamic and rewarding opportunity to work with people experiencing homelessness, supporting them with compassion, purpose, and professionalism.
You’ll be part of a multidisciplinary team that’s committed to trauma-informed, culturally safe, and person-centred practice. Your role will be pivotal in ensuring service users receive timely support, resources, and advocacy while they await case management. You’ll help keep the wheels turning - managing referrals, preparing resources, and supporting the team to deliver consistent, high-quality services.

SKILLS AND EXPERIENCES:
Key Role Responsibilities
Manage incoming referrals and conduct initial assessments. Provide advice and triage support to clients awaiting case management. Monitor client vulnerability and escalate concerns as needed.
Maintain accurate and timely data in the client management system. Support reporting and compliance activities in collaboration with leadership.
Purchase and prepare client resources such as welcome packs to support case management activities. Maintain inventory and coordinate with Case Workers on resource needs.
Support the coordination and documentation of team meetings.
Collaborate with Case Workers, other Support Workers and team members across the Program directorate to share knowledge and ideas and provide support to develop the internal support network.
Essential requirements
Certificate IV or Diploma in a human service focused discipline or; substantial experience, skills and expertise in case management in a community services position.
Demonstrated experience supporting people with complex needs in a community services setting.
Effective verbal, written and interpersonal communication skills.
A current Western Australian Driver’s Licence and First Aid Certificate
Ability to obtain a Western Australian Working with Children Check, National Police Clearance, a Form 395 Department of Communities Clearance
