Responsibilities

In this role, you will:
· Provide person-centred support to adults with diverse needs, including but not limited to autism, learning disabilities, mental health conditions, physical disabilities, and sensory impairments.
· Collaborate with service users to create and implement tailored support plans, focusing on their unique aspirations, strengths, and areas of development.
· Assist with daily living activities, including personal care, meal preparation, and medication management.
· Foster a supportive and inclusive environment that promotes independence and well-being.
· Work closely with colleagues, healthcare professionals, and external agencies to ensure holistic and coordinated care for service users.
· Maintain accurate records and documentation in accordance with regulatory requirements.
