We provide aged care, retirement living and home care services for older people, with community services including mental health, disability, youth and family, and domestic and family violence support. Our team provides one-on-one in-home support and community participation services for people living with a disability in the Kingaroy & South Burnett region.

ABOUT THE ROLE:

As a Support Worker, you will play a vital role in enhancing the well-being of individuals with disabilities by providing tailored support to help them live independently, engage in the community, and develop essential life skills.

RESPONSIBILITIES WILL INCLUDE:

• Delivering creative and engaging individual or group activities
• Supporting clients in achieving personal and developmental goals
• Assisting with daily living tasks, including personal care and meal preparation
• Providing in-home and community-based support to promote independence
• Encouraging skill development through mentoring and positive role modelling
• Collaborating with families, caregivers, and other professionals to ensure high-quality care
• Maintaining accurate records and documentation in compliance with guidelines
